The molecule was approved, the combination arm was not
Pfizer closed TACTIVE-U sub-study B, pairing the PROTAC ER degrader vepdegestrant with ribociclib, after 20 Phase 1b participants and for stated strategic business reasons. Six months earlier the same molecule won FDA approval as monotherapy, but only in the ESR1-mutated subgroup the combination arm had not been enrolling.
Mechanism Risk Score
| Component | Points |
|---|---|
| Phase-weighted failure burden | 5.1 / 30 |
| Archetype severity | 2.0 / 25 |
| Temporal recency | 4.3 / 15 |
| Genetic evidence deficit | 8.5 / 15 |
| Programmatic saturation | 15.0 / 15 |
For ESR1 in ER-positive, HER2-negative advanced or metastatic breast cancer, the Mechanism Risk Score is 35/100 (yellow band). The score is a failure-burden index derived from Claidex post-mortems on this target–disease pair, not a probability of approval.
Yellow. Saturation is at ceiling (52 clinical-stage programmes against ESR1) and carries the score. The genetic deficit term is low because ESR1 has direct human genetic support in ER-positive breast cancer. Risk here is competitive and portfolio-driven, not target validity.
This score does not predict whether the next trial will succeed. It flags how heavy the documented mechanistic failure record is before a new program is justified.
What was tried
TACTIVE-U sub-study B tested vepdegestrant, an oral PROTAC estrogen receptor degrader, in combination with the CDK4/6 inhibitor ribociclib in ER positive, HER2 negative advanced or metastatic breast cancer. Pfizer was the lead sponsor, with Arvinas Estrogen Receptor, Inc. as collaborator. The design was an open label, sequential Phase 1b/2 arm inside a larger umbrella study, with no masking or randomisation.
Participants had at least 1 percent ER stained cells on their most recent biopsy, one or two prior lines for advanced disease, and exactly one prior CDK4/6 inhibitor based line. Phase 1b used an mTPI-2 escalation and de-escalation scheme to find a recommended Phase 2 dose, opening with a seven day vepdegestrant lead-in. Cycles ran 28 days, with vepdegestrant daily and ribociclib for 21 days on and 7 off.
Three primary endpoints were registered: dose limiting toxicity counts in Phase 1b, objective response rate in Phase 2, and ribociclib exposure with and without vepdegestrant in a drug interaction cohort. The arm opened on 1 March 2023 and completed on 6 October 2026. It closed at 20 participants, and the sponsor was unusually explicit about why: "Enrollment was permanently closed for strategic business reasons after 20 participants enrolled in Ph 1b. The decision was not related to safety, efficacy, benefit-risk, or participant welfare. The planned Ph 2 and DDI components were not initiated."
The biological hypothesis
ESR1 encodes the estrogen receptor, a ligand dependent nuclear receptor that drives proliferation in ER positive breast cancer. It is among the better genetically anchored targets in oncology. Open Targets scores the ESR1 to ER positive breast cancer association at 0.435, built from a genetic association component of 0.592 and a clinical component of 0.495.
A PROTAC degrades rather than blocks. Vepdegestrant tethers an ER ligand to a cereblon binding element, so the receptor is ubiquitinated and routed to the proteasome. That matters because ESR1 mutations acquired under aromatase inhibition produce a constitutively active receptor with reduced affinity for competitive antagonists. Removing the protein sidesteps the affinity problem that blocking it runs into.
The combination rationale was specific. Work published in Clinical Cancer Research in 2024 showed vepdegestrant was efficacious as monotherapy and with CDK4/6 or PI3K and mTOR pathway inhibitors. Degrading the receptor and inhibiting CDK4/6 hits two sequential nodes of the same cyclin D to CDK4/6 to Rb axis, the standard argument for endocrine plus CDK4/6 backbones in this disease.
What actually happened
Twenty participants enrolled in the Phase 1b portion. The Phase 2 portion and the drug interaction cohort were never initiated, and no results have been posted. A parallel arm of the same umbrella, testing vepdegestrant with samuraciclib, closed the same way after 11 participants.
The molecule itself did not fail. While this arm sat in Phase 1b, the monotherapy programme succeeded on a narrower front. The FDA approved vepdegestrant as Veppanu on 1 May 2026 for ER positive, HER2 negative, ESR1 mutated advanced or metastatic breast cancer progressing after at least one line of endocrine therapy. The approval rested on VERITAC-2, which randomised 624 adults, of whom 270 had ESR1 mutated tumours. In that ESR1 mutated population, median progression free survival was 5 months with vepdegestrant against 2.1 months with fulvestrant, a hazard ratio of 0.57 with a 95 percent confidence interval of 0.42 to 0.77. Objective response was 19 percent against 4 percent. Overall survival was immature, with 16 percent of deaths recorded at the time of the analysis.
The shape of that approval is the relevant fact. A 624 patient trial produced a label covering the 270 patient subgroup.
Safety provides no competing explanation. In openFDA FAERS, vepdegestrant carries 4 reports, 3 of them serious, with neutropenia the most frequent term at 2, which is what a drug approved six months ago looks like in a spontaneous reporting system. Ribociclib carries 6,642 reports with neutropenia top at 1,334.
Failure mechanism, best guess
This was strategic reprioritisation, and for once the posted reason and the independent evidence agree. The asset's commercial identity narrowed during the trial's life, from a broad endocrine backbone combinable across the ER positive population to a monotherapy for an ESR1 mutation defined subgroup after endocrine therapy.
That narrowing stranded the sub-study. Its enrolment criteria were written for the wide indication, at least 1 percent ER staining with no ESR1 mutation requirement. Once the label landed on the mutated subgroup, a combination arm recruiting an unselected population was no longer reading out against the group the drug would be sold into. Phase 2 would have produced a response rate in a population the label did not cover.
Crowding compounds it. Open Targets lists 51 clinical stage molecules against ESR1, including elacestrant and camizestrant, which are approved or near approval and pursuing similar combinations. A programme entering that field without a biomarker hypothesis of its own has a weak differentiation case regardless of its pharmacology.
The biology here is unrefuted. Nothing in the record tests whether ER degradation plus CDK4/6 inhibition works, because the portion designed to answer that question never opened.
How to prevent this next time
No endpoint level data was posted for this arm, so the available levers are qualitative. Three apply.
The first is biomarker alignment between a combination sub-study and the pivotal trial it depends on. This arm enrolled on ER positivity while the pivotal study was accumulating a biomarker defined benefit. Had it enrolled on ESR1 mutation status, its result would have been interpretable whichever way the label fell, and it would have survived the narrowing that killed it.
The second is a pre-agreed decision rule tying an umbrella sub-study to the pivotal readout it hedges. This arm ran Phase 1b for three and a half years without an explicit go or no-go.
The third concerns the drug interaction cohort, a separate casualty. Ribociclib exposure with and without a cereblon directed degrader has value independent of whether the combination is commercialised, and that small cohort could have been completed and published at modest cost.
The single highest leverage change would have been to enrol the combination sub-study on the same ESR1 mutation status that the pivotal monotherapy trial was using to define its primary population.
What this means for similar programs
The Claidex mechanistic risk score for ESR1 in ER positive breast cancer is 35 of 100, in the yellow band, from a phase burden of 5.1, archetype severity of 2.0, recency of 4.3, genetic deficit of 8.5, and saturation of 15.0.
Saturation is at its ceiling and carries the score. The genetic deficit term is low, the correct reading of a target with direct human genetic support and an approved degrader. The risk at ESR1 is not that the target is wrong. It is that 52 programmes compete for the same biology, and one that cannot name the subgroup it wins in is exposed to portfolio decisions it does not control.
Open questions
Whether ER degradation adds to CDK4/6 inhibition beyond what either does alone remains untested here. Whether the 20 participants dosed in Phase 1b will have safety and pharmacokinetic data published is unknown, as is whether the ESR1 mutated restriction widens as overall survival matures in VERITAC-2.
